BAY de NOC COMMUNITY COLLEGE PERSONNEL POLICIES AND PROCEDURES Originator: Board of Trustees Revision Date: Subject: Work Areas 609 WORKERS' COMPENSATION - INJURY AND ACCIDENT REPORTING POLICY Prompt Reports are Imperative. We are required by law to furnish our insurance carrier with prompt reports of all injuries involving employees during the course of their employment. Your cooperation is the best insurance. The following procedures provide you with directions for reporting all work related injuries. 609.1 Report all injuries immediately to the Business Office for documentation and return necessary reporting forms as soon as they are completed. 609.2 Form CS-104 (Authority for Treatment) is given to the employee to surrender to the doctor. This form is to be returned to the Business Office by the doctor. 609.3 If the injury is very serious call the Business Office and give them the information over the phone. The form can follow in the mail. 609.4 For CS-246 (Employee Report of Injury). When possible the employee should complete the E.R.I. form. This serves as written narrative of the alleged injury. Michigan law requires all public employers with more than seven (7) employees to keep records of occupational injuries and illnesses in a manner outlined by the rules at each work establishment. The following are the procedures at Bay de Noc Community College for complying with this law: 609.5 When an emergency occurs, faculty and staff are expected to intervene by calling for an ambulance to assist anyone on campus or at an off-campus class, who appears in need of such assistance. 609.6 If possible, faculty or staff will stay with the person who is ill or injured and direct a student, staff or other adult to call for the ambulance. Faculty and staff should ensure that the person calling know the appropriate phone number, room number, and building where assistance is needed. 609.7 If the teacher must leave the classroom, he/she shall stop all class activity unless suitable class supervision is available. 609.8 Faculty and staff shall be aware of how to call the local ambulance. The numbers are as follows: Escanaba: Dial 9 for an outside line on-campus, then 911 for assistance. Dickinson County: 774-1114 - ambulance service 774-1313 - hospital (will also send ambulance) Manistique: 341-2133 - ambulance service 341-2163 - hospital (will also send ambulance) Marquette: 475-9911 - Central Dispatch 609.9 If possible, faculty or staff shall ask someone to wait outside to direct the paramedics to the person requiring assistance. 609.10The ambulance shall be called in the following situations: a. Whenever an individual loses consciousness b. Epileptic seizures, insulin/diabetic reactions. c. Serious complaints of illness: chest pain, fainting, severe abdominal pain, etc. d. Falls with serious injury e. Serious cuts or burns f. Any other situation, that causes you alarm regarding the immediate health and safety of the individual. 609.11Faculty and staff shall not agree to monitor a person who wants to sleep after a seizure or some other type of incident. One seizure may be a prelude to subsequent seizures. Medical assessment and intervention is appropriate, even if the person refuses care from the ambulance paramedics. Allow the paramedics to assess the situation and assist the person in reaching a decision about transport and intervention. All conscious, competent adults have the right to refuse transport and care. 609.12 The college will assume costs for an ambulance visit which is refused. 609.13 In an emergency, call the ambulance before calling the nursing department for assistance. Nurses may or may not be able to come depending on the day and time. Calling the nursing department first wastes valuable time in an emergency. 609.14 Faculty are not required to allow a seriously sick or injured student to remain in class. Faculty or staff may assist the student in making arrangements to return home or to seek their own care. 609.15 Anyone sustaining a minor injury should be encouraged to seek his or her own medical care. 609.16 Each supervisor is to inform all employees that they are to report all injuries. 609.17 Any employee, who suffers a work-related injury or illness regardless of how minor, shall report the injury or illness to the immediate supervisor at the earliest opportunity. 609.18 All supervisors are to make a record of the injury or illness. A copy of the record shall be submitted to the Business Office as soon as possible. Administrators have the responsibility to keep all appropriate records. 609.19 The Business Office shall keep all submitted reports on record for inspection. |
