OUTSTANDING STAFF AWARD
NOMINATION FORM
(DEADLINE: FEBRUARY 28, 2017)

Beginning February 1 of each year, nominations will be accepted for Outstanding Staff Awards. Nominees may consist of administrative, support staff, full-time, or part-time employees as defined in the definition of Policy 203 Hiring. Anyone who has been chosen to receive the Outstanding Staff Award will be ineligible to receive the award again for a period of three years.

Please complete the following information on the nominee of your choice. The online form is submitted by clicking the Submit Outstanding Staff Award button at the bottom of the form. Paper copies of the form are available at the Student Services Reception Desks.
